Re:                  JV and Varsity Soccer
 
JV(4^th and 5^th Grades) tryouts will be held on Wednesday, 8/22 from 3:30-4:30 and Varsity (6^th-8^th Grades)tryouts will follow from 4:30-5:30.  All students trying out for Soccer must return this form to me before their tryout.  The following forms must be submitted to me before the student is able to try out.  These forms can be found on the school website.

1. Consent and Release from Liability (Diocese of Palm Beach)
2.  Physical (needs to be up to date)
3. Concussion Release Form
4. Heat Related Illness Release Form
